Read across the whole ledger, 79 of 86 checked sellers pass S4 — the highest rate of any criterion in the Standard, and the least informative. 39 of those passes belong to research vendors that sell no compounded drugs at all. Among the 47 checked sellers for whom the disclosure is a disclosure of something, 40 make it and 7 do not.

Criterion S4, compounding disclosure, asks a seller of compounded drugs to say so plainly, and to state that compounded products are not approved by the Food and Drug Administration. The census below was run against Standard v1.1, on the date stated above, over every record in the ledger.

Why the whole-ledger number should not be quoted

79 of 86 is a true count and a misleading summary, and the reason is visible in the failing side's denominators. 39 of the 44 research vendors pass S4, and not one of them fails it. Their bases say why: they sell no compounded drugs and print research-use labeling instead. A rule that says “if you sell a compounded drug, disclose it” is satisfied for free by anyone who sells none.

That is not a flaw in the criterion, which is written conditionally on purpose. It is a flaw in quoting the criterion's overall pass rate as though it measured candor. Read that way, this field looks close to fully compliant on the single disclosure that matters most to someone about to inject a drug their government has not evaluated. It is not.

The sellers who do not say it

A disclosure on some pages is not a disclosure

S4 is recorded against the pages a buyer would actually reach, which is why a record can fail while carrying the sentence somewhere. A seller whose treatment pages mostly say a drug is compounded and not FDA-approved, but whose pages for the two best-selling molecules do not, has left the disclosure off the path most of its buyers walk. The basis in each case names which pages carried it and which did not, so the reader can judge the gap rather than take the disposition on trust.

Why the question is not decorative

A compounded drug is not a generic. It has not been reviewed by the Food and Drug Administration for safety, effectiveness or manufacturing quality, and that is a fact about the product rather than an opinion about the seller. A buyer who believes they are receiving an approved medicine is wrong about the most consequential thing in the transaction, and the sentence that corrects them costs a seller nothing but a line of text. The criterion asks only for that line.
